Dear Parent or Custodian:
With your permission, Harvest Hill would like to provide transportation for your child(ren) to our children's and youth programs on Wednesday nights.
Children will be picked up between 5:45pm and 6:15 and will be returned home between 8pm and 8:30pm.
Parent/Custodian Statement:
I give consent for my child(ren) named above to ride to and from Harvest Hill Wednesday night services. Further, I agree to hold Harvest Hill, it's officers, workers or representatives guiltless in the event of accident or injury to my child(ren).
I further grant permission for Harvest Hill workers to act on my behalf in the event of accident or injury to my child(ren) provided reasonable effort has been made to reach me. By signing below, I affirm that my child(ren) can leagally be transported on a bus in Missouri.
